Records Requests
The District fully complies with the California Public records Act (CPRA) as well as the California Health and Safety Code for public and patient records requests.
In compliance with the health and safety code if a patient or guarantor of a patient would like to review personal medical records the District will not charge for this service. If a patient or guarantor of a patient would like to receive a copy of personal records the District will provide these records also at no cost to the patient or guarantor.
For law firms representing patients for potential litigation the District charges $15.00 for medical records and $15.00 for billing records. If a law firm is requesting both medical and billing records a single check can be used in the amount of $30.00 per patient, pursuant to California Evidence code §1158. The check should be made out to Exeter District Ambulance.
Firms can mail payment and records requests as well as a Health Insurance Privacy Protection Act (HIPPA) release form signed by the patient to the District office at:
- 302 E Palm St Exeter, CA 93221
For all other records requests that fall under CPRA the district does not charge for reviewing any records available to the public under this act but will charge a fee of $0.25 per page of the records that a member of the public would like to receive copies of pursuant to California Government Code §7922.530(a)
